ADMINISTRATION OF MEDICATION

It is the responsibility of the parent/guardian to bring any medication to school and remove any unused medication when treatment is complete. All prescription medication must be brought to school in the original container. The pharmacy label must have the following information:

Name of Student

Prescription Number

Name of Medication and Dosage

Administration Route or other Directions

Date

Licensed Prescriber's Name

Pharmacy Name, Address, and Phone Number

 

All non-prescription medication must be brought to school in the original manufacturer's labeled container with the ingredients listed and the child's name affixed to the container.

 

The school nurse will discard any unused medication that is not picked up by the parent or guardian. Only medications brought from home, prescribed for the identified student, and accompanied by a signed permission sheet from the parent or guardian will be administered to that student.

 

ALL MEDICATION INCLUDING BOTH PRESCRIPTION AND OVER THE COUNTER MEDICINES MUST BE TURNED INTO THE OFFICE AS SOON AS THE STUDENT ENTERS THE BUILDING.
